 It would be good to change the Squid lens to do the following:

   * resolve refresh_pattern into fields
   * "extension_methods" should be parsed into a list, similar to how
 sshd.aug parses each AcceptEnv line into a list of env vars
   * break entries upgrade_http0.9 and broken_vary_encoding down similar to
 what http_access does
