Alpine and speakup

In Alpine's configuration tool, there is an option called
'show-cursor'.  Once this option is selected, Alpine will put a cursor
in that speakup can track.  Since you won't be able to navigate the
configurator without cursor being on, you can go and do 'ctrl+w', and
look for 'cursor'.
