[EnMasse] EnMasse on kubernetes

Hi,

Just want to announce that EnMasse now runs successfully on Kubernetes 
as well as OpenShift[1]. This hopefully makes it available to many more 
users. There were some changes to the address-controller and the 
configuration files in order to support it, mainly around templates and 
routes.

I wrote a quick guide on how to set it up, that only covers AMQP and 
non-TLS setup so far[2].

For now, it looks like kubernetes does not supports pure TCP nor SNI + 
TLS passthrough for Ingress resources "out of the box", so some manual 
steps are needed to expose the messaging endpoints.
