I don't have any multipath experience with ext3, so I hope that's not an
issue here.

Well, did you run e2fsck on the filesystem?

[Yamin, Yossi] Upgrade to Linux Advanced Platform 5.2 Fixing this
problem is mentioned on the release notes  

Is this reproducible or did this only occure once?
