There are 2 things that are puzzling me about this:

First of all, having a nice web form to ask for free media is great.
Couldn't we have that in the Fedora Infrastructure, instead of spreading
it in each countries ? This would give a bit more credibility to the
program cause "Hey, I don't know who those vglug guys are, I want a Fedora
media, can I trust them ?" ;)

It looks like there's already something more central on the wiki:
https://fedoraproject.org/wiki/FreeMedia

Adding a nice webapp with a form to automate the procedure that is
described on this page could be much better than doing this stuff each one
in a different location.

Second, we're talking about Free Media program right ? Not about the
vendors who want to distribute Fedora for profit (correct me if I'm
wrong).

However, the site specifies:
"We will be charging you for the media and for shipping and handling
expenses. (CD-Rs.15, DVD-Rs.25, S&H-Rs.35)"

Charging for the shippingand the media ? o_O

The same wiki page says:
"Fedora Free Media Program is a volunteer initiative under  Fedora
Distribution Project that aims to distribute Fedora Media (DVDs) for free
to individuals who can't afford to buy or download."

This one says:

"#  Limit to only 1 DVD per request per person per release to cut cost and
time.
# Please note this is a volunteer initiative by local Fedora Ambassadors
or Contributors who are willing to burn DVDs and ship them with their own

expect them to send out an average of 2 DVDs a week.
# Your Free Media request might not be fulfilled at this time as this
program depends on their own time and expenses."

So it looks to me that your site should be more something like
https://fedoraproject.org/wiki/Distribution/OnlineVendors

Maybe that's what you wanted to talk about, but as the subject of the mail
is "Submit your request for Free Fedora DVD",...
