Example of modules.alias:

alias pci:v00001077d00004022sv*sd*bc*sc*i* qla4xxx

That is for Qlogic iSCSI HBA.

Files needed on DD: rhdd, modules.cgz, modinfo, modules.alias and
modules.dep
