This one is the one that bites me the most as a package maintainer....
especially for istanbul :->
There are problems I simply can't reproduce, and if I can't reproduce them
myself its very difficult for me to use my time effectively as an advocate
to getting the issue resolved with aid from upstream. I feel pretty
strongly that the person interfacing with upstream needs to be a person who
is or has experienced the problem directly or we're just wasting time trying
to diagnose a problem we can't reproduce.

I tend to lump 'feature requests' into this category... and sadly I tend to
sit on these... and I shouldn't. I need to do a better job of encouraging
upstream developers to look over the feature request tickets for ones they
like. Or maybe I just be honest and admit that I hate every feature that
has been so far proposed on any package I maintain and I'm not brave enough
to close the bugs out as BADIDEA.
