No, you're misreading this. If a package with some n-v-r was never built
it doesn't make any sense to have a special changelog entry for it. The
wiki is talking about small changes in already built packages. Only once
the package has been built and you do even a trivial change (or even no
change at all except bumping release because for example the dependecies
changed) and do rebuild, then you must add a changelog entry.
