prompts in command examples

If this is based on the feedback that a lot of users copy-paste the
commands into their shells and receive an error stating that the prompt
shouldn't be in, then it makes sense to remove the prompt and add
comments/narration.

However, this does bring out a separate point altogether - aren't users
expected to *not* do copy-paste and type in the commands (for better
learning etc etc) ? :)
