This uses --includedir and --libdir to specify install locations and create
/etc/ld.so.conf.d/openmpi.conf to point to the library directory.  This gives us
some funky directory structures, but oh well:

/usr/include/openmpi
/usr/include/openmpi/mpi.h
/usr/include/openmpi/mpif.h
/usr/include/openmpi/openmpi
/usr/include/openmpi/openmpi/ompi
/usr/include/openmpi/openmpi/ompi/mpi
/usr/include/openmpi/openmpi/ompi/mpi/cxx
/usr/include/openmpi/openmpi/ompi/mpi/cxx/comm.h

/usr/lib64/openmpi/openmpi/mca_allocator_basic.so

I've also submitted a patch for lam, bug #178967

I'm currently testing with the patched lam.  The more eyes on this the better
though.
