Pam question maybe?

I'm almost positive it was hacked. I saw login attempts, then I started
getting inode errors. but I didn't actually see anything wrong. I formatted
and reinstalled after trying to fix with fsck, but then system just wouldn't
boot. Now I want to add disabling login attempts on the new system.
