If you were looking at the device contents with Nautilus then fam may be
monitoring the mountpoint.  Killing nautilus may release it.  Killing
fam (as root) would release it.

lsof will explain which programs have which files open:
  lsof /mnt/flash

Sometimes it may be handy to do this instead:
  lsof | grep /mnt/flash
