I was having a very similar problem on a large SDSL installation; The
provider controlled the router. As soon as we closed off the unneeded
ports the connectivity improved dramatically. It's a long shot but worth
a try.

Once that is resolved, one way or the other, I think that squid is a
great idea. We keep this block list in sync weekly:
