Qmail's architecture is pretty good, but it's not suitable for the real 
world without those patches.  I usually suggest that people just use 
Courier.  It's very familiar if you've used Qmail, but it's actively 
developed and supports a wealth of features, without having to wade 
through a bunch of patches.
