The last time I saw this problem it turned out to be (apparently) a CD
drive.  Make sure the drive can be "Master" on the IDE, it apparently is
bootable, but probably first try another CD drive (they are cheap, easily
obtainable, and fairly easy to change out).

In my case I changed out the CD drive and never looked further into the
problem.
