pcmcia card woes

I've got a D-Link DMF-560TXD PCMCIA 10/100 NIC-modem combo which has
worked fine for me for several years, however in recent kernels, it
won't work at all when inserted into any notebook.  When inserted, I
see the following in dmesg:

pccard: PCMCIA card inserted into slot 0
cs: memory probe 0xde000000-0xde0fffff: excluding 0xde000000-0xde00ffff
pcmcia: registering new device pcmcia0.0
