One thing you might want to check is whether your user_prefs file
is consistent with the version of SpamAssassin you're running.
A colleague had problems similar to what you're describing
(headers not being altered), and the problem turned out to be
syntax that had been copied over from an old configuration file
and was no longer being recognized.
