I'm a bit more familiar with the nvidia driver and the logs that it produces....

A while back, over 2 years ago, I had a similar problem. I had a SyncMaster
172T and a ViewSonic VG800b and an Nvidia based card with an analog and
digital output. When using a single monitor, I pretty sure I found myself
using only the analog. But, when I went to a dual head setup the digital
out worked just fine. Then, I finally got the digital working by itself.

Anyway, how about sending your xorg.conf as well. It may jog my memory.
And, if possible, the Xlog with the nvidia driver. Did you happen to try
running nvidia-xconfig in run level 3 and use the xorg.conf it generated?

I believe that you are able to get the login prompt at level 3, right?
