RAID drive failed, but SMART shows no errors?

Not neccessarily. SMART data is also not always reliable so consider it
an indicator and no more.

You need to tell the kernel politely first preferably. See the SCSI howto
which I think covers /proc/scsi/scsi and add/removing devices.
