I'm not Endi, but I will add my bit:

Usually I'm very frustrated from various confirmation dialogs.

IMHO it should be configurable per-user. Each user is scared from different 
actions :-) Per-user setting will allow to enable confirmation only for 
"dangerous" operations, as a user feels it.
