Did you try 'ipactl start' afterwards?

I'm not sure what has caused 389-ds database issue but from the log 
excerpts it looks like 389-ds was able to fix those.

Fedora 16 stable updates got freeipa 2.1.4-5 and 389-ds 1.2.10-rc1 
tonight.
