Looks likely the database is corrupt. When is your most recent backup that would include the ldap database directory? Default is /var/lib/ldap - check "directory" line in /etc/openldap/slapd.conf

Or if you are lucky the problem is an index that should be rebuilt. We can research the error and get to resolution later.

The first thing to do is preserve what you can. If ldbm database is corrupt then you don't want any write activity which may make things worse. Edit /etc/openldap/slapd.conf, changing the "readonly off" line to "readonly on" then do this:
service ldap stop
tar -cvzf ldbm.backup.tgz /var/lib/ldap
service ldap start


That way you have a snapshot of the ldbm files for a safety, and you put the ldap server in readonly mode to block write attempts, which also makes it safe to export the contents with slapcat.
slapcat -l mydirectory.ldif
or if that fails try
slapcat -cl mydirectorywitherrors.ldif


Start with that and I'll post more shortly.
