Since your error seems to be with TFTP, why not try increasing the
verbosity of TFTP's logging to syslog?  A more verbose tftp log might
show you where things are going wrong (it's helped me in the past when
I've had tftp server issues, though it wasn't with LTSP specifically).

You'll want to take a look at the tftpd man page, and then make the
necessary changes to /etc/xinetd.d/tftp.  Restart xinetd after you've
made your changes.  Then tail /var/log/messages while a thin client is
booting.  When you're done, don't forget to turn the verbose logging
back down.
