Bike-shedding won me over - I'm renaming this test server-death.c, and
will push soon with that rename.

I still have a few more patches to post hopefully later today adding
*_notify interfaces. I know you want to cut a release soon, but I'll
leave it up to you whether to do so after this lands, or whether to also
make the release include *_notify.
