Noticed this (a bunch of it) in the log:

>   WARNING: PV /dev/sda2 in VG VolGroup00 is using an old PV header, modify the VG to update.

I don't know if we ultimately perform any operation that modifies the volume group, but if we do, it seems plausible that that update prevents the LVM driver in RHEL5 from recognizing the volume group again :(
