Yes, you are using it incorrectly.  Because this is being done as an XML
namespace, besides adding the XML elements that you want, you also need to
add the namespace to the top of the XML:

<domain type='kvm' xmlns:qemu='http://libvirt.org/schemas/domain/qemu/1.0'>
  ...
  <qemu:commandline>
    <qemu:arg value='-ctrl-grab'/>
  </qemu:commandline>
</domain>
