[libvirt PATCH v5 1/9] internal: Add CONCAT macro

Using the two-step idiom to force resolution of other macros, e.g.:

  #define bar BAR
  CONCAT_(foo, bar)     // foobar
  CONCAT(foo, bar)      // fooBAR
