[linux-lvm] patches to get dm working in 2.5.45

There are 3 patches that are needed to get dm working in 2.5.45, they
can be found here:

One patch was an ommission (vcalloc), one is due to set_device_ro
parameters changing in 2.5.44, and the final one fixes a bug due my
misunderstanding of the get_gendisk function.
