[lvm-devel] git mirror in the works: first step: user-map

I'm setting up a git mirror of the lvm2 cvs repository.  The first step is to
create a user-map file mapping the user names used in cvs commit messages
to the ("User Name", <user-email at example.com>) pair that git records.
Here's my first cut:

FYI, the git repository will be sync'd once or twice (it's efficient)
per hour from cvs, and will otherwise be read-only.
