Password changes, Shadow, and libpwdb

Ok, I've got a VERY strange problem and wonder if anyone can
assist. Below is my pam.conf for passwd and pwdb.conf, this all
works for about a day, then suddenly, when someone tries to change
a password they start getting the following:

New UNIX password:
Retype new UNIX password:
passwd: Critical Error - Aborted

My logs show the following:

Feb 18 11:33:12 shell1 PAM_pwdb[22419]: user (rharby/708) update failed; pwdb: internal failure - seek help

Now, keep in mind when I first set this up, users could change their passwords
no problem for about 24 hours, then this started happening.

Oh this is all using the following rpms:

pam.0-54-4
pwdb-0.53-2

Heres my pam.conf section for passwd
------------------------------------
passwd auth required /lib/security/pam_pwdb.so
passwd account required /lib/security/pam_pwdb.so
passwd password required /lib/security/pam_cracklib.so
passwd password required /lib/security/pam_pwdb.so use_authtok shadow
passwd session required /lib/security/pam_pwdb.so

Heres my pwdb.conf
-------------------
user:
        unix + shadow

group:
        unix + shadow

Any help here would be great.. if I change my pam.conf to the following,
it all works fine:
--------------------------------------------------------------------------
passwd auth required /lib/security/pam_pwdb.so
passwd account required /lib/security/pam_pwdb.so
passwd password required /lib/security/pam_unix_passwd.so
passwd session required /lib/security/pam_pwdb.so

Looks to me like a problem in libpwdb, so I didn't know where to
mail this, either here or the libpwdb mailing list. Please Help :)
