vlock is not xlock, the entry you displayed in /etc/pam.conf is not what
is getting used for vlock. This is obvious from the fact that pam_warn is
sending info to syslog and pam_warn is not on your xlock entry. My guess
is that you are defaulting to the OTHER entry which likely is using
pam_deny.so as well as pam_warn.so.

Try adding the following to you /etc/pam.conf file:

vlock auth required /lib/security/pam_pwdb.so

BTW: You shouldn't need the "shadow md5" parameters, they are used by the
passwd service only
