more pam_mkhomedir.so problems

I get the following when I attempt to log into a RHEL4
server with AD credentials:

I have the right entry in the system-auth file:
> session required pam_mkhomedir.so skel=/etc/skel/ umask=0022

I'm a little confused as to why it is trying to create /etc/skel/,
and then why the session open is failing? Is pam_mkhomedir just
fundamentally broken on RHEL4? I'm running pam-0.77-66.13, which
seems to be the latest and greates according to up2date. The same
pam config line works just fine on SuSE 9.3 and 10.

Any insight would be appreciated.
