nptl 0.10

Not too many changes which are generally useful.  I fixed two minor bugs
which probably nobody hits so far, and started with the x86-64 port.
Those interested in ports to x86 < i686 should look at the
i486/lowlevelrwlock.S changes.  This is how these kind of changes should
be made if they are not extensive.  An i386, if somebody is interested
in ancient history, would need completely different files due to the
lack of the basic instructions used.
