I flushed out the bottom of that page to indicate the possible
permutations of configurations, CA certificates, and entitlements that
need to be covered in testing. I also added some notes on generating
your own CA and entitlement certificates (including how to add the
entitlements into the certificates).

This is a pretty complicated setup for testing, so any
comments/questions are appreciated. There are a lot of "if A and B but
not C" sorts of scenarios, so let me know if it's not clear to read.

Note: I'm still not 100% done with the code, but this should get QE
started on test case creation.
