I don't know for sure if that's what they're doing, but I suspect it
since Brad's machine seems to be exhibiting the classic "SMTP access
denied" behaviour and it _used_ to work.

I think that "EX_USAGE" is the wrong error to return in these
conditions.  It should be "EX_UNAVAILABLE" (69), "EX_PROTOCOL" (76)
or "EX_NOPERM" (77), although that last one may be deceptive as well.
