Keep losing RAID device

The drives are on 2 x 2 channel Promise Ultra100 IDE cards, each drive has
it's own channel.
I don't think the motherbaord is relevant, but it a 2.4 GHz Intel processor
and an Asus MB of some kind.
Three of the drives work fine, it is only one that has this problem.
RAID 5 works with three or more drives.
