perl-DBD-MySQL

i have redhat 9.0 serive and an apache server on it.
i trid to user perl to develop a registration system.
i have installed mysql apache and perl-DBD-MySQL-2.1021-3.i386.rpm.

when i click http://coopunit.forestry.uga.edu:8080/book/regstr.html
i get an error page.

the error log message is

any i do find
/usr/lib/perl5/vendor_perl/5.8.0/i386-linux-thread-multi/auto/DBD/mysql/mysql.soexits.
