There are 2 way to do this

1. You should buy a RHEL ES subscription and register yourself into RedHat Network
   When you logon to the RHN you will see a red bar on the top, then you gonna go to channels
   After you will see a list and you will find Red Hat Enterprise Linux ES (v. 4 for 32-bit x86) link and click it.
   There you will see Downloads link when you click to the link you will get ES 4.0 and ES 4.0 Update 1 ISOs

2. You should buy a RHEL ES subscription and approximately you will pay 50 Euro or something to get original
   RHEL ES 4.0 cd pack. But for this you should talk to your RHEL distributor in your country or directly contact with
   RedHat. But cd packs are not update packs forexample when you buy a CD pack for RHEL ES 4 you will have 
   RedHat Enterprise Linux ES 4.0 not ES 4.0 Update 1
