You are correct, I usually disable sendmail by default on all the hosts - but in this case - it looks like - it needed to be running in order for mail relay to work.

sendmail? So that should be sendmail.cf? Or postfix, in which case it would be main.cf?
I run sendmail in daemon mode (/etc/sysconfig/sendmail-->DAEMON=yes
assuming Red Hat or Centos) and have a DS macro in sendmail.cf pointing to my mail relay (Exchange) host. I think that's all I did. The access map and local-host-names are default, as is the Cw macro.
Don't recall if I tried setting it up through submit.cf without daemon mode.
