What's eating my swap space..?

I've noticed that with kernel 2.4.1-0.1.13 (who dreamt up that naming,
btw?) the system doesn't seem to free up swap space. More and more is used
up, and eventually the system slows down and I have to reboot. Very
un-Linux that.

How could I diagnose this?
