Once I figured (and remembered) to switch from tulip to de2104x, my network is up:
# lspci
00:06.0 SCSI storage controller: LSI Logic / Symbios Logic 53c810 (rev 02)
00:07.0 ISA bridge: Intel Corp. 82378ZB/IB, 82379AB (SIO, SIO.A) PCI to ISA Bridge (rev 84)
00:0b.0 Ethernet controller: Digital Equipment Corporation DECchip 21040 [Tulip] (rev 24)
00:0d.0 VGA compatible controller: S3 Inc. ViRGE/DX or /GX (rev 01)

# cat /etc/modprobe.conf
alias scsi_hostadapter sym53c8xx
alias eth0 de2104x

# netstat --interfaces=eth0
Kernel Interface table
Iface MTU Met RX-OK RX-ERR RX-DRP RX-OVR TX-OK TX-ERR TX-DRP TX-OVR Flg
eth0 1500 0 9035 0 0 0 8323 0 0 0 BMRU

# ethtool eth0
Settings for eth0:
Supported ports: [ TP AUI ]
        Supported link modes: 10baseT/Half 10baseT/Full
        Supports auto-negotiation: No
        Advertised link modes: 10baseT/Half 10baseT/Full
        Advertised auto-negotiation: No
        Speed: 10Mb/s
        Duplex: Half
        Port: Twisted Pair
        PHYAD: 0
        Transceiver: internal
        Auto-negotiation: on
        Current message level: 0x000000f7 (247)
        Link detected: yes

The speed and duplex are dependent on the peer (i.e. switch port), but the link is up and that's what's important
right now.
