[linux-lvm] LVM VG keeps disappearing

It is only a sugestion.

lvm version.
fdisk for all patitions
vgscan -vv in verbose mode and debug mode (if you has complied with debug)
pvscan -vv
lvscan -vv
df -k
mount

all information that you can!
