So what is special about the systems that are missing? How do the
differ from the other CentOS 5 servers that end up in the list? Is it
different arch? Different set of packages installed?

Since for some systems the application works as expected, there is
likely something in the data about the affected profiles that triggers
the wrong behaviour.
